It was a heartbreaking moment for Elizabeth and it apparently had a profound effect.
Later on, she learned that Jake is really Jason. She came close to exposing all right there on stage. For once, a character pointed out the practical concerns involved with a switch/coverup like this. Namely, small things like DNA and fingerprints. It makes the truth inevitable, and he figured it was best to just be done with it. Elizabeth, on the other hand, felt differently. ‘General Hospital’ spoilers indicate she saw it as a “deal with that if/when it happens” moment. It is a bit of a drastic thing from the Elizabeth we’ve become accustomed to seeing. But it does show a common weakness amongst characters.

Ultimately, Elizabeth’s selfish/cruel choice came out of frustration and desperation. Apparently, she’d been given the short end of the stick so many times, she couldn’t take it again. With Jason unaware of who he really is, they could finally have their chance at happiness.
General Hospital spoilers hint that Hayden Barnes’ (Rebecca Budig) part in the deception might have been the final straw. The way she saw it, people were scheming and cheating people all the time with no consequences. In fact, they got what they wanted usually. Elizabeth wanted that for herself, and she got pushed far enough to resolve she’d do anything to get her own happiness. Sometimes, you just have to look in the mirror and see what kind of monsters you might have created.
